Six longitudinal-care programs from one infrastructure: RPM, RTM, CCM, PCM, BHI and IBT. Two of them need a connected device; four do not, which means a practice can begin before a single device ships. The Programs page sets out what each one requires.
Often, but not freely. RPM and RTM cannot both run for the same patient in the same period, and neither can CCM and PCM. Each program needs its own consent and its own eligibility, and the same minutes cannot count toward two programs. SureMediks enforces these rules rather than leaving them to the claim.
Documented patient consent captured before the program starts; clinical appropriateness judged by you; qualifying time recorded as the work happens rather than reconstructed afterwards; for device programs, enough days of transmitted data in the period; and documentation meeting the standard for that specific service.
No, and be wary of anyone who does. Eligibility, coding, coverage and applicable requirements vary by patient, provider, payer and program, and clinical and billing responsibility remains with the practice. Reimbursement is an outcome of care delivered and documented — it is not the product.

Nobody has to. Devices are provisioned and shipped ready to use — the patient steps on the scale and the system does the rest. We handle connectivity, replacement and technical support to the patient by phone and in-app.
